What came before the company

Five years beside the community. Lived experience, NHS relationships and hard-earned trust through Plasma of Hope.

Hospital discharge / from ward to welcome home

The first hour home should not feel like the hardest one.

We help make the first hours and days feel safe, settled and properly supported.

03 / An ordinary day

Care fits into life.
Not the other way around.

The best support protects the small routines that make a day feel like your own.

Morning / on your terms

Ready for the day, your way.

Personal care with dignity, patience and choices kept intact.

Afternoon / still connected

A reason to go out—or someone to stay in with.

Conversation, errands, appointments and companionship that never feels forced.

Evening / everyone can breathe

Reassurance for the whole family.

Respite, overnight and live-in support from a team that knows the person, not just the plan.

Sometimes it is a few hours.
Sometimes it is the difference between coping and living.

04 Lived experience

Sickle cell & thalassaemia

Care that does not ask you to explain your whole life before it can understand you.

Our specialist pathway grows from Plasma of Hope’s lived-experience leadership and five years supporting people affected by sickle cell and thalassaemia.

We understand our community because we are part of it.

05 A careful future

Our Hospital at Home roadmap

Building clinical capability in the right order.

Our phased model protects quality: growing from enhanced home care into nurse-led support and, ultimately, full Hospital at Home partnerships.

  1. 1

    Launch pathway

    Enhanced home care

Personal care, discharge support, companionship and non-clinical chronic illness support.

    Months 0–6
  2. 2

    Next

    Clinical home support

Nurse-led visits, wound care, monitoring and post-surgical recovery after the required approvals.

    Months 6–12
  3. 3

    Future

    Full Hospital at Home

Virtual ward technology, rapid response and structured clinical pathways with NHS partners.

    Months 12–24

Clinical and regulated services will only begin once the appropriate CQC registration, governance and qualified team are in place.

06 Why MJN Care

Professional care.
Personal understanding.

01

Community before company

A Community Interest Company with an asset lock: surplus supports community health, rather than leaving it.

02

Culture is part of care

Language, family, food, faith and lived experience are not extras. They shape how safe, heard and respected a person feels.

03

Connected from day one

Built from an established community network and working relationships across health, care and the voluntary sector.

07 Close to home

Serving the Black Country

Local care grows from local trust.

We are beginning where our relationships already run deep, then expanding carefully across the wider West Midlands.
