A family and care professional helping an older man settle back at home

Support for the day you are living—not a routine imposed on it.

Flexible, compassionate help to live safely and confidently in your own home.

Explore support

01 Your life, your care

Good care should fit around the person—not shrink the person to fit a service.

We begin with who you are, what matters to you and what a good day looks like. Then we shape dependable support around that.

For some people, that means help getting ready and preparing a favourite meal. For others, it means a safe return from hospital, regular companionship, or time for a family carer to rest.

How we can help

Care for the moments that matter.

01

Personal care

Respectful support with washing, dressing, hygiene, continence, medication prompts and daily routines.

  • Led by your preferences
  • Consistent care team where possible
  • Clear care plan and family updates
02

Hospital discharge

Transport, settling in, welfare checks and practical support for a safer first few days back home.

03

Companionship

Meaningful visits, conversation, errands and support to stay connected with everyday life.

04

Respite & overnight

Short breaks for family carers, overnight reassurance and planned live-in support.

05

Long-term condition support

Non-clinical monitoring, routines and practical support for people living with sickle cell, diabetes, COPD or heart failure.

02 Starting care

Four gentle steps from first call to familiar support.

No pressure. No assumptions. A clear conversation at every step.

  1. 1

    We listen

Tell us what is happening now, what you are worried about and what you want life at home to feel like.

  2. 2

    We meet

A home assessment helps us understand routines, preferences, risks and the people around you.

  3. 3

    We plan together

You receive a clear care plan, agreed visit times and a team matched to your needs.

  4. 4

    We keep learning

Regular reviews keep care useful, personal and responsive as life changes.

Clear about where we are

Safe growth comes before fast growth.

MJN Care is developing its services through a phased Hospital at Home model. Personal care will begin only when CQC registration and all launch safeguards are in place. Nurse-led and clinical services will follow only with the relevant approvals, governance and registered professionals.
