A nurse and young man reviewing a health reading together at home

Support shaped by the people who know the journey from the inside.

Culturally informed care at home, built from lived experience—not assumptions.

Talk to someone who understands

01 More than a diagnosis

Pain is not the only burden. Not being believed is one too.

Through our sister organisation Plasma of Hope, our leadership has spent five years listening to and standing beside people living with sickle cell and thalassaemia.

That experience shapes a different kind of support—one that recognises the whole person, understands cultural context and helps families navigate life before, during and after a hospital admission.

Our developing pathway

Support on both sides of the hospital door.

Services will grow carefully as our registrations, clinical team and NHS pathways develop.

At home

Staying well

  • Routines and wellbeing checks
  • Medication prompts
  • Family and carer support
  • Education and advocacy
In transition

Coming home

  • Planned discharge support
  • Settling-in visits
  • Post-admission recovery
  • Connection with care teams
Future clinical phase

Hospital at Home

  • Nurse-led assessment
  • Clinical monitoring
  • Crisis support pathways
  • Haematology integration

Clinical monitoring, medication administration and crisis treatment are future services and will only begin with the required CQC registration and NHS clinical governance.

When care understands the life around the condition, trust arrives sooner.

MJN Care approach

01

We believe people

Symptoms, pain and previous experiences are listened to with respect.

02

We include families

Carers receive practical understanding without taking agency away from the person.

03

We connect the system

Our long-term aim is joined-up support with haematology, primary care and virtual wards.

Training & consultancy

Help your team understand sickle cell beyond the textbook.

We are developing lived-experience-led awareness and cultural competency sessions for healthcare teams, employers and schools.
